FOR:              BOYS Grades 1st thru 7th (grade you will be entering)\r\n

\r\n

\r\n

Each camper will receive a Badger Athletic T-shirt.\r\n

\r\n

Emphasis will be placed on the fundamentals of basketball, with each camper receiving as much individual attention as possible. Team games will also be included.\r\n

\r\n

Please fill out the lower portion of this form and return it as soon as possible, so we can include you in our camp.  Keep the upper portion of this letter as your reference. Send application to \r\n
